We want your first visit to be an enjoyable experience. To help facilitate a smooth, efficient first visit, please read the “What to Expect” section below and complete all registration forms through your Phreesia check-in link before your visit. In addition to the registration forms, extra questionnaires will need to be completed on the patient portal. The portal questionnaires must be completed prior to your child’s appointment, as they will be discussed during the visit. If forms and questionnaires are not completed ahead of your scheduled visit, please arrive 20-30 minutes before your scheduled appointment. Some forms can also be completed online. What to Expect
All visits are by appointment only. Well Child appointments (including well baby, school, camp and sports physicals) are scheduled several weeks in advance. Sick appointments are scheduled on a same day basis. Consultations for behavioral and ADHD concerns or for complicated medical issues are scheduled for an extended appointment time if you provide this information to the staff.
Bring all paperwork that you need us to complete for schools and daycare including health forms, medication forms, emergency treatment forms. Give these to the nurse at the beginning of your appointment so that they can be completed during the appointment. There is a $10 form fee for any forms not completed during the visit.
Bring pertinent medical records including your child’s growth chart and all previous immunization records from the previous physician’s office or clinic. Do not assume they have been sent by the previous physician.
We request that you bring your insurance card to each visit and to notify us if there have been any changes. We require your co-pay at the time of the visit.
We have two waiting rooms—one for WELL and one for SICK. Please make sure you’re on the proper side for the type of appointment you’ve scheduled.
We provide minimal toys to keep the spread of germs to a minimum. Bringing your own toys from home will be helpful. We do provide reading materials in the interest of promoting reading.
